       February 1, 2008:  New Passport Forms

    • February 1, 2008:  As of February 1, 2008, the two-parent consent requirement has been extended to all children under the age of 16 when applying for passports for minors.  Both parents must appear (with child/ren) and sign for children under 16.  If only one parent can appear with the child/ren, please follow the consent requirement instructions.

    • JANUARY 31, 2008
      U.S. and Canadian citizens will need to present either a WHTI-compliant document, or a government-issued photo ID, such as a driver’s license, plus proof of citizenship, such as a birth certificate. DHS also proposes to begin alternative procedures for U.S. and Canadian children at that time.

    • August 2006:  Effective immediately, all passport applicants must provide their correct social security numbers in box number 5 of the passport application form, or otherwise indicate that you have no social security number. Applicants who fail to provide a valid social security number may be subject to a US$500 penalty enforced by the Internal Revenue Service (IRS).

    • New Fee for Certificates of Registration issued to Non-EEA Nationals, effective as of May 27, 2006.

    • Effective September 26, 2005 the U.S. Passport Service will no longer amend valid passports.
