Replacing Lost and Stolen Passports
If your passport has been lost or stolen, you must appear in person during our opening hours at the Passport Office to apply for a replacement. You should bring:
- photographic identification;
- your U.S. birth certificate, naturalization certificate and/or previous U.S. passport (if available);
- two passport photographs;
- and $100.00 fee ($85.00 for a child under 16).
- Complete either version of form DS-0011. DO NOT SIGN THE FORM: (This form can also be obtained when you appear at the Embassy).
-- DS-0011 Application for U.S. Passport or Registrations (Fillable) [ 1001 Kb]. This form should be filled in online and printed on white A4 or letter-size paper, with each page on a separate sheet of paper.
-- DS-0011 Application for U.S. Passport or Registrations (Non-Fillable) [ 743 Kb]. This form should be downloaded and printed on white A4 or letter-size paper, with each page on a separate sheet of paper. Complete in Block Letters.
(Please ignore pages one and two of the instruction pages of this form, as they do not apply when applying for your passport outside the United States. Instead, please follow the instructions on this page.) - In the case of a stolen passport, please submit a police report if possible.
- Self-addressed, REGISTERED ENVELOPE; (in the absence of a registered envelope, your passport will be sent to you by regular mail). Please allow four weeks to receive your passport by mail.

Please Note: All passport applicants must provide their correct social security numbers in box number 5 of the passport application form, or otherwise indicate that you have no social security number. Applicants who fail to provide a valid social security number may be subject to a US$500 penalty enforced by the Internal Revenue Service (IRS).
