Lost, Stolen or Mutilated Passports

The American Citizen Services Unit in Dublin can replace lost, stolen or mutilated passports with a valid passport upon verification of your identity and citizenship.  An emergency, limited validity passport may be issued for immediate travel the day you apply.  You must bring proof of travel: airline ticket or electronic ticket if your departure is imminent.  If you will not be traveling within two weeks you may be issued a full-validity passport instead.

To replace a lost or stolen passport, you will need to complete Form DS-11 and form DS-0064. Please type or print legibly in black ink when completing all sections of this application. Please print out and submit only one-sided pages of the application.  Do not sign it until told to do so.

Note that the online version of the application form includes instructions on the backside written for use in the U.S., which do not apply to your application in Ireland.


Please follow specific instructions provided below:

In order to obtain a replacement passport, ALL applicants, including children, MUST appear in person at the Passport Office located at the Consular Section of the U.S. Embassy with the following items:

  • A police report substantiating the loss or theft, if available;
  • All available identification such as: birth certificate, driver's license, photocopy of the missing passport, or an expired U.S. passport;
  • Two (2) identical, recent color photographs taken full face, with a white background, measuring 5 cm by 5 cm (2 in x 2 in). Irish photo booth photographs are not acceptable. Please ask the photographer for photos for a U.S. passport otherwise you may be given the wrong format.
  • If your departure is not imminent, a new full validity passport(s) will be returned by mail.  Please include a stamped self-addressed envelope, size 6in x 9 1/2in approx., for the return of your passport.  Please affix to the envelope a €5.50 regular postage stamp or a combination of stamps to the value of €5.50.  We will return your passport to you by Express Post mail. For imminent departures, limited-validity passports will be issued immediately.  Once we have received the completed passport application, it takes approximately two weeks for you to receive your new passport.

All applications for children under 16 require the consent of both parents or legal guardians of the child.

If one parent/guardian is unable to sign the application form DS 11, he/she MUST submit a notarized statement of consent authorizing the other parent to make the passport renewal application for his/her child on his/her behalf.  Please click here to download this Statement of Consent: Issuance of a Passport to a Minors under 16. You must also submit an original or legible photocopy of the child's birth certificate.

The fees for a replacement passport for an adult (over 16) are $100, or the euro equivalent for adults, and $85, or the euro equivalent for children, payable by credit card or cash in U.S. dollars or euro only. Personal checks are not accepted. Exact change is preferable if paying in cash.

When traveling, please remember to carry a photocopy of your passport separately from your passport. In the event of loss or theft, the copy will facilitate issuance of an emergency passport.

Security at the Embassy

Please do not bring any electronic devices, such as laptop computers, Blackberries, iPods, or large bags, such as backpacks, suitcases or packages to the Consular Section as there are no storage facilities on Embassy grounds. Security guards may hold small tourist cameras and cell phones until your departure, but there is no storage available for any other large items or electronic devices — please do not bring them at the time of your appointment.
